Non-emergency medical transportation operations

Spell NEMT Pro. Run every trip from booking to billing readiness.

A tenant isolated operations platform for non-emergency medical transportation. Scheduling, traffic aware route planning, dispatch, driver navigation, service evidence, compliance workflows, and operational reporting in one governed platform.

  • Web dispatch and native field app
  • Embedded Google Maps navigation
  • Governed CRM and ERP connections
  • Role scoped and tenant isolated

Why NEMT operations fragment

Booked in one place. Assigned in another. Documented by phone call.

Most transportation operators run booking, dispatch, and proof of service across separate tools, then reconcile them with calls, texts, and paper. Dispatchers sequence several vehicles around pickup windows, appointment deadlines, mobility requirements, shifts, and traffic that will not hold still. Managers usually learn about a late trip when the facility calls.

Generic fleet tools do not model the passenger lifecycle. Generic CRM tools do not dispatch, route, or run the field. Spell NEMT Pro keeps the whole journey on one record, from the first inquiry to the evidence finance needs.

Six moves, one record

Book, plan, dispatch, navigate, document, hand off.

Every stage writes to the same trip. Nothing is retyped, and nothing is reconstructed later from memory.

Book

One way and recurring trips with pickup windows, appointment times, service type, facility, payer, authorization, and a fare snapshot that later rule changes cannot quietly rewrite.

Plan

A daily plan across multiple driver and vehicle pairs, including take-home vehicles that start from restricted operational bases.

Dispatch

Capability and conflict checks, ranked driver and vehicle recommendations, and protected state transitions so assignment never silently dispatches a trip.

Navigate

Embedded Google Maps turn by turn guidance in the driver app, with road snapped routes rather than a straight line drawn between two pins.

Document

Service attestations covering actual pickup and drop-off, odometer readings, mileage, wait time, passenger acknowledgment, and exception reasons.

Hand off

Completed work becomes billing ready evidence with a stable idempotency key, so a retry cannot create a duplicate financial document.

The governed lifecycle

Every state change is an event with an owner.

Unsupported jumps are rejected. Accepted transitions record the actor, source, timestamp, and optional location.

01

Requested

The trip is captured with passenger, route, service type, and payer context.

02

Scheduled

A driver and vehicle are assigned after capability and conflict checks pass.

03

Dispatched

Approved work is released to the driver in its planned order.

04

En route and arrived

The driver app reports progress, with GPS during active assigned work.

05

Passenger on board

Pickup evidence, odometer, and mobility context are recorded in the field.

06

Completed

Drop-off, wait time, and acknowledgment close the trip and open the billing handoff.

Route optimization

The plan is generated. The decision stays yours.

The optimizer weighs pickup windows, appointment deadlines, stop pairing, loading time, wheelchair and stretcher needs, driver and vehicle compliance, capacity, authorizations, shifts, deadhead distance, workload balance, and traffic aware travel time.

Optimize day

Generates a draft plan with metrics, warnings, and a stated reason for anything it could not assign.

Approve assignments

A dispatcher accepts the allocation. Nothing reaches a driver on the optimizer's say-so.

Release to drivers

A separate, deliberate action publishes the approved work to each driver.

Protected work

A configurable frozen window shields active and imminent trips, so a passenger already on board is never reassigned underneath the driver.

Compliance and evidence

Requirements that reach dispatch, not a document folder.

Configurable policy packs cover a federal baseline, Texas Medicaid and DRTS, or your own requirements. Spell NEMT Pro supports configurable compliance workflows; it is not a substitute for legal advice or payer enrollment.

Driver and vehicle evidence

Licensing, exclusion screening, driving history, drug policy, background checks, training, insurance, registration, inspection, maintenance, and ADA equipment, each with issuer, reference, and expiry.

Optional dispatch blocks

Administrators choose whether missing, expired, or invalid evidence should hold a trip. The policy stays organization controlled.

Service attestations

Authorization, payer and program, actual times, start and end odometer, wait time, acknowledgment or exception reason, and driver confirmation.

Cases and registers

Incident, accident, complaint, grievance, and safety records with severity, immediate action, and authority notification, exportable to Excel and PDF.

The field app

Drivers see their work. Nothing else.

A native iOS and Android experience for supported phones, scoped to the driver's own assignments.

Ordered manifest

Today and upcoming work in sequence, with pickup and drop-off, passenger contact, and mobility context.

In-app navigation

Live location, maneuver and voice guidance, ETA, distance, and traffic aware rerouting, with the destination following the trip state.

Shift controls

GPS supported clock in, break and resume, and clock out, with active trip background location where tenant policy requires it.

Offline safe

A mutation queue holds supported field actions through poor connectivity and replays them idempotently when the signal returns.

Private notifications

Assignment and operational alerts that keep passenger names and addresses off the lock screen.

No admin surface

Driver accounts cannot see unassigned trips, another driver's work, or post evidence against a trip that is not theirs.

The Spell ecosystem

Licensed on its own. Connected when you want it.

NEMT Pro operates as its own transportation workspace. Connections extend the relationship and financial lifecycle without blurring who owns what.

NEMT Pro owns operations

Trip lifecycle, dispatch, GPS evidence, route plans, service attestations, and billing ready evidence.

Spell CRM owns the relationship

Passengers can link to CRM Leads and Contacts, and facilities and payers to Accounts, without a second identity record.

You choose the financial authority

CRM Billing, Spell ERP, or an external accounting system. NEMT Pro does not create competing official invoices.

Spella stays advisory

Operations briefs use anonymized facts only. Passenger identity, addresses, coordinates, and medical notes are excluded, and people approve every dispatch, fare, and compliance decision.

FAQ

Common questions.

Is this an emergency transportation system?

No. Spell NEMT Pro is designed for non-emergency medical transportation. It is not a 911, emergency dispatch, ambulance clinical, or medical decision support system.

Can we use NEMT Pro without other Spell modules?

Yes. NEMT Pro is independently licensed and entitlement gated. Optional CRM and ERP connections extend the relationship, workforce, and finance lifecycle without mixing operational ownership.

Does the optimizer assign trips automatically?

It generates auditable, traffic aware route recommendations across eligible drivers and vehicles. A dispatcher reviews the plan, approves assignments, and separately releases work to drivers.

Does it support drivers who take vehicles home?

Yes. Authorized administrators can configure restricted operational bases and effective driver and vehicle pairings. Exact base information is kept out of passenger views, ordinary exports, and lock screen notifications.

What happens when connectivity is poor?

The app retains an offline safe mutation queue for supported field actions and uses idempotent replay to reduce duplicate updates when connectivity returns.

Can compliance requirements block dispatch?

Administrators can configure requirements and choose whether missing or invalid evidence should block dispatch or completion. Policies remain organization controlled.

Does it create invoices?

NEMT Pro creates governed billing ready evidence. The financial system you select remains authoritative for official invoices, receivables, payments, and ledger entries.
